Mr Nicholas Raoul Francesco Lagattolla BMed Sci (Hons), BM BS, FRCS, FRCS (Gen) has a legal duty to explain how he uses any personal information collected about you, as a registered patient at his practice, as the practice needs to maintain personal records about you and your health and the treatment you receive in both electronic and paper formats.
We will collect non-sensitive information such as personal details, including name, address, a record of appointments, e-mail address and telephone number. We require this information contractually in order to provide our service in accordance with article 6(1)(b) of the GDPR-EU legislation. We will also collect sensitive data when it is relevant to enable us to deliver effective therapeutic care. This is in accordance with article 9(2)(h) of the GDPR-EU legislation.
We may also hold information about you from your GP or another practitioner who might write to us on your behalf in order to refer you.
Your data is collected for the purpose of providing direct patient care; however, we can disclose this information if it is required by law, if you give consent or if it is justified in the public interest. We will share information about you with your GP or your referring practitioner only if it is relevant and believed to be beneficial to your health.
We are committed to maintaining confidentiality. Your data is stored securely and is protected.
We will retain your records for a period of 5 years after treatment has ended.
You have a right to access the information we hold about you. Furthermore, should you identify any inaccuracies in that information, you have a right to have the inaccuracies corrected. You also have the right to have your personal data restricted or deleted if you feel it is being used improperly, unless it has to be retained by law.
The Information Commissioner’s Office (ICO) is the overall regulator. They may be contacted on 0303 123 1133. You have the right to complain to them if you feel there has been a breech regarding the above.
